Overview
The purpose of the State Tax Business Registration form is to have a client sign off on SDP registering for a client's State Tax Account on their behalf. This form is used to provide the Tax Department pertinent information such as the main Contact Information, Company Information, Tax Registration Information, and login information for the applied State Taxes. This process is completed manually by the Tax Department at a Fee of $200.00 per State requested.
CSR Expectations
The Expectations from Customer Service when a client requests SDP to register their company with a specific Tax Agency is that the full attached form is filled out completely (unless otherwise specified to keep blank). If there is any missing information, the Tax Department may send the CRIS Ticket back and request the client to fill out the form again.
Once the State Tax Business Registration form has been filled out completely and signed by an Authorized Contact of the company, attach the Registration form to your CRIS Ticket and email the client the following for moving forward with assistance from the Tax Department:

Note - During Quarter and Year End tax filing months, response turnaround time is increased to 15 Business Days.
